Plug in a microphone and speakers, or a headset etc, and make sure you have a default mic and speaker option set (yes I'm serious).

I have this same problem. I attempted to make a dedicated server and selected internet. That was not working so I closed the program with task manager. Now my multiplayer will start to open, ask me if I want to run in safe mode, then it will show the splash image and close. I have reinstalled Call of Duty 4 and made sure the sound drivers are up to date. I have a mic plugged in and everything. It was working fine before I tried to make the dedicated server. Oh and my singleplayer works fine.

Ah nevermind I fixed it. All I had to do was swap in a new config_mp and it worked like a charm.
