4 possible things:

1. Netgear = shit. Get a Billion or Draytek modem.
2. GPU failure
3. Network congestion at night time
4. Noise on your phone line, this is fixed with Telstra or Optus (depending which hardware you are on)

Don't ask me to elbaorate, google it or call Telstra/Optus.

xuse i have the EXACT same issues as you.
Optus cable internet, game looks fine from my point of view but in killcams or w/e i warp like a bitch.
Even if i am the only computer/application using the internet i still warp.
Here is some evidence that optus cable is the worst possible internet choice for online gaming, especially fps's.

I do get lag spikes from 11am - 11pm, and my ping is never under 100 during these times. Which forces me too play late at night, i still warp at night but not as much.
I have tried other configs and i still warp all over the place.
I am now 100% sure that it is the optus cable and not my game settings, router, modem or what ever have you.

I am going to change isp and post back to let you know if that fixes these above issues....
